This problem seems to come and go at random, there was a time where every loading screen was a guaranteed disconnect and then a month or so of flawless play, recently this issue has resurfaced. It's not as bad as it used to be, but I stillf requently get dissconnected on loading screens, always in the same way.
The loading bar reaches about 45% then slows down dramatically, before reaching the 50% mark and dropping me. After that happens, logging back in and getting the character list back up usually takes between 30second and a minute, but has gone up to at least 2 minutes before, though that is rare.
Bottom line is this: Playing the game or rather enjoying the game is very difficult when every loading screen has a high chance of dropping me, for no apparent reason. I know my own connection is not an issue, it is rock solid in every possible way except in this specific instance.
Is this a known issue? Is there a reason for this happening? If so, is there anything I can do or is this something unheard of? At any rate, I felt it was about time to report it.

you can try disabling on demand patchin in the launcher option. Then instead of downloading patches as needed while in game (on instance changes) it downloads the complete patch on startup.
if you have a marginal connection on demand patch can delay map loading so you end up timing out.

Thanks anyway though, I'll check and see if on demand patching is on.

The voice chat might bring some problems too, may be disable it and try it without...
