Items I am a bit confused

Some Item that drop state buff a certain skill then right below that state is is enhancing no skill. What are these items? I do not understand how to get these items to enhance a skill.

Also what do items look like that replace powers and unlock costume parts are they a certain color or state something in there description?

It means they work on a skill you dont have.

And I believe they will only list what skills they are enhancing when they are actually slotted. For most, you should be able to figure out what types of skills it enhances from the other info.
